Youth Matches – the reports must be submitted within 24 hours of the end of the match and the report emailed to:

Report Writing – Only the current USSF forms shall be used. Both reports must be completed detailing the reasons for misconduct, injury or unusual incident. Only report the facts. Consult with your ARs to verify all information prior to departing the match and record their contact information. Time is of the essence, especially in youth matches. If you have any questions about what to do or how, contact you ARA.
Be sure you check the league or tournament requirements for the game because some leagues or tournaments may have additional reporting requirements.
